Recognizing the importance of social needs to overall health status, there has been a trend toward screening for social drivers of health (SDoH) in many health and community-based organizations. In fact, the Centers for Medicare and Medicaid Services (CMS) made such screening a requirement for all in-patient encounters as of January 1, 2024.

Why has this become so important? What does this trend portend? Is it just more regulation, or does it provide a new kind of opportunity? Can it provide an avenue for true population health management and value-based care? Can systems connect more elements of the community to provide a real, maybe even “smart”, continuum of care? Could this be an avenue toward a connected health community that has not been answered by health information exchanges? What are the uses of SDoH data? What is being done in the Kansas City area?
